Please arrive about 10 minutes early so that you have enough time to fill out a New Patient form. To save some time, you can download the form below and fill it out prior to your appointment.
Initial Examination
Your dentist will begin by reviewing your medical and dental history. They will then perform a complete dental exam. This will include a thorough assessment of your gums, teeth, bite and jaw. He will also do an oral cancer screening and X-rays if necessary.
Treatment Plan
Your dentist will develop a long-term dental treatment plan customized to your needs based on your examination, dental history, and oral health goals.

The full cost of treatment must be paid at the time of your appointment. We accept cash, debit or most credit cards.

How do I cancel or reschedule an appointment?
To cancel or reschedule an appointment, please call us. By notifying us as soon as possible of any changes, we will be able to offer this schedule to another patient. If canceled within 48 working hours, fees apply depending on the duration of the appointment.

In the days before your appointment, you will receive automated phone, text or email appointment reminders from us. If you need to cancel or reschedule an appointment, please call us as soon as possible to let us know.
